Add an inner shadow effect to the frame layer with the following settings. The Contour is a custom contour that I made. To recreate the contour click on the one showing in the window. Now duplicate the contour below.

Add a bevel and emboss effect to the frame layer with the following settings. The Gloss Contour is a custom contour that I made. To recreate the contour click on the one showing in the window. Now duplicate the contour below. Now click Texture in the left column. Click the pattern showing and choose the dirty cracked pattern that I made. Use the settings below.

Add a new layer and link and merge it with the frame layer. Now add a drop shadow effect to the frame layer using the settings below. Now add a bevel and emboss effect to the frame layer using the settings below.

Open the image you'd like to add to the frame and bring it into your frame's file. Name the image's layer picture and place it under your frame's layer in the layers palette.

Click on the frame layer and hide its effects. Take the lasso tool and go around your frame making selections where the cracks are and then deleting. When done show the frame layer's effects.

With the frame layer as the active layer, take the magic wand and click down inside the frame. Go to Select>Modify>Expand...>2. Click on the picture layer and choose Layer>New>Layer via Copy. Hide your picture layer. And you're done. To save this signature first hide the back color layer and then use the save as a PNG-24 setting found here, Transparent background without a matte.
