START With A New File. Fill Layer 1 with white and name it white back. Your final signature will have a transparent background but we added a white layer because it's easier to see as you're working. This is the color I used for my slide. To get this color click inside the foreground color's box in the toolbox. In the Color Picker window enter the settings I added a red outline to and click OK.

Add a new layer. Select the custom shape tool and choose the rounded rectangle shape. Using the settings you see below hold the shift key while you click and drag.

Select the rectangular marquee tool and change the Style to Fixed Size in the options bar. Enter the amounts you see below for the Width and Height. Click on your file and it will automatically make a selection 133px x 90px. If you want to move your selection, although it doesn't matter where it is when you fill it, just click and drag inside the selection. Make white your foreground color. Now add a new layer and name it white. Choose option + delete for Macs and alt + delete for Windows to fill the selection. Deselect.

Add a new layer. Make black your foreground color. Change the Width and Height amounts in the options bar to the ones below and make your selection. Fill the selection then deselect. Name this layer black.

Add a vertical guide. Make sure your guides are set to snap. Use the move tool to center the white and black layers along with Layer 1 with the guide.

Make the white layer a selection. Click on Layer 1. Now choose delete to make the shadow at the top of the opening. Deselect.

Change the settings again in the options bar for the rectangular marquee tool. Make a selection where you'll see your picture through. This selection does need to be in the right place before you delete so if you need to move the selection click and drag inside the selection. Click on Layer 1 and choose delete. Then click on your white layer and choose delete. Finally click on your black layer and choose delete. Deselect.
