Start with a new file 5" x 2" at 100 ppi with a transparent background. Fill the layer with white and name it white back. You have this layer so it's easier to see as you work your final signature will have a transparent background. Make a new layer and name it black shapes. With the custom shape tool using the settings you see make your shapes. Your hearts and ovals will be filled when you make your shapes this picture was taken after I've deleted the inner sections.

To delete the inner sections of your hearts use these settings to make a Work Path. Click and drag inside all the hearts you want to delete the inner sections of. I lowered the opacity of the black shapes layer so it was easier to see the path.

Go to your paths palette and click Load path as a selection, red outline. With your black shapes as the active layer click delete. Deselect. Now using the first settings posted for the custom shape tool make your inner hearts. To delete the inside of your ovals take the elliptical marquee tool and make a selection in one of the ovals then choose delete. Repeat for the remaining ovals.

Take your magic wand and click inside the oval that will have your name. Choose Select>Modify>Expand>3. Make a new layer and name it black filling. With black as your foreground color choose option + delete for Macs and alt + delete for Windows to fill the selection. Deselect.

Apply these two layer effects to the black shapes layer. To do that click on the layer and then click on the tiny f in a circle at the bottom of the layers palette.

Make a new layer and put it above the black shapes layer. Name it pattern. Make the black shape layer a selection. With the paint bucket tool selected change the Fill to Pattern in the options bar. Click where the purple outline is to see your patterns. From the pop up window I chose this pattern.

With your pattern layer as the active layer click anywhere in the selection to fill it with the pattern. Deselect. Change the opacity of the pattern layer to 10%.

With a very light grey as your foreground color take your type tool and write your name. Apply this layer effect to it. Use the move tool to position it in one of the ovals.

Make a new layer and name it shine. Put this layer at the top in the layers palette. With the elliptical marquee tool make a selection like you see in the picture. Choose the gradient tool and click inside the gradient in the options bar, red outline. Choose the Black, White grad and click OK. In your selection click at the bottom and while holding the shift key drag to the top of the selection and release, blue arrow. Deselect. Change the setting to Screen for the shine layer.
