Start with a new file. I worked at 5" x 2.5" at 100 ppi with a transparent background. Name Layer 1 frame. With the rectangular marquee tool make the selection for your frame. If you don't know how check out this thread, Frame Selections Make black your foreground color and fill the selection by choosing option + delete for Macs and alt + delete for Windows. Deselect.

Apply these effects to the frame layer. You do that by clicking on the tiny f in a circle at the bottom of the layers palette and choosing them. To change the Contour and Gloss Contour click the arrow on the right side of the contour showing in the Layer Style window and select the contour from the pop up window. If you hold your cursor over a contour a tiny window with its name will pop up. If you don't see the Peaks Contour and Valley-High Gloss Contour then click the arrow in the top right corner of the contour window and load your other contours. To change the color click on the color box in the Layer Style window then in the Color Picker window that pops up enter those RGB amounts in the RGB fields.

Make a new layer and fill it with black. Name this layer black back.

Select the custom shape tool from the toolbox. Use these settings in the options bar. To choose your shape click on the Shape that's showing in the options bar, in this picture that's the one on the far right, then select your shape from the custom shapes window. To see how I made the heart shape I used check out the tutorial Modifying Custom Shapes.

Click on your frame layer in the layers palette so it's the active layer. With black as your foreground color click and drag to make your heart shape. To move the heart shape as you're making it hold the space bar. You want it to slightly overlap your frame. If you hold the shift key while you click and drag your heart will be in proportion. Now make the second heart placing it in the top right corner of your frame.

Select your Horizontal Type Mask Tool. If it isn't showing in your toolbox click and hold on the arrow in the lower right corner of your type tool and choose it.

Type your name(s). If you're familiar with using Quick Mask Mode the red mask will be familiar. If you need to change the font or its size, just like when you use the type tool simply drag around your name and make your changes. You'll notice a type layer isn't automatically created in the layers palette. This is because you're making a selection.

Click the check in the options bar and you should now see the selection.
