Start with a new file 5" x 2" at 100 ppi with a transparent background. Fill the layer with black and name it black back. You fill it by making black your foreground color and then selecting from the menu bar Edit>Fill... foreground color. Or you can use your paintbucket tool to click in the file. Make a new layer and name it pattern. Make sure this layer is above the black back layer in the layers palette. Select your paintbucket tool and in the options bar choose Pattern as the Fill. Now click in the pattern shown in the options bar to see all of your available patterns. To load any patterns you don't see click the tiny arrow in the top right corner of the pattern window. Select a pattern and click in your file. To change the color of the pattern choose the brush tool and in the options bar set the Mode to Color. Choose a pink and color over your pattern.

Take the rectangular marquee tool and make a long thin selection.

Make a new layer and name it line. Choose pink as your foreground color and select Edit>Fill...foreground color. Deselect the selection and add this layer effect to the layer. You do that by clicking on the tiny f in a circle at the bottom of the layers palette and choosing it.

Make a new layer. Click on your line layer and link and merge the new layer with it. You do this because we will be using a bunch of different angles for the layer effects and if you notice when you change the angle of an effect for one layer it also changes the angle on your other layers with an effect.

Make a copy of your line layer by clicking and dragging it to the new layer icon. Take your move tool and move it down. Now make a copy of this layer. For this new line layer choose Edit>Transform>Rotate 90˚ CCW. Take your move tool and move it to the left of your signature. Make a copy of this layer and move it to the right. You should have four lines.

Link and merge the top and bottom lines and name the layer horizontal. Link and merge the left and right lines and name the layer vertical. Make sure the vertical layer is under the horizontal layer in the layers palette.

Now apply this effect to the horizontal layer. For the shadow's color I clicked where the blue arrow is pointing and the Color Picker window popped up. Then I chose from my swatches palette the pink I used for my lines. Then I clicked and dragged the hollow circle down, see white arrow. When I use drop and inner shadows I sometimes like to use a darker version of the color they will be appearing on instead of using black. Here's the color I selected and then I hit ok.

Link and merge the horizontal and vertical layers and name the layer bars. Open your picture's file. Click and drag the Background from the layers palette over your siggy file and release. Reduce or enlarge your picture and use the move tool to place it. I named this layer teddy. Make sure this layer is under the bars layer. Take the rectangular marquee tool and use it to make a selection and delete any parts of your pup that go beyond the bars.

Apply this Inner Shadow to your pup's layer, so my teddy layer.
