Make a new layer and name it inner shadow putting it under your just (your dog's name) layer. Now choose Edit>Fill... with foreground color selected. Deselect. With the elliptical marquee tool make a selection like in the picture and click delete. Deselect.

With the inner shadow layer as the active layer change its opacity to 50% in the layers palette. Now choose Filter>Blur>Gaussian Blur with this setting...

Make a new layer and name it rings. Using the same gold color you used to make your oval frame make your rings with either the brush tool or the elliptical marquee tool. I made them with the Ring brush. To make the gold your foreground color you can either enter the settings in the Color Picker window like you did earlier or take your eyedropper tool and click down one time on the color in your siggy.

I hid some of the layers so you could see the back shape. Put this shape on a new layer naming it faded. Notice where it's located in the layers palette. Change its opacity to 20% in the layers palette.

Make a new layer naming it tiny spirals and add your spirals. I used this brush to make them... Make another layer named corner ring. This is the shape in the lower right corner. You can use the brush tool or elliptical marquee tool to make it. I made it with the Ring brush. Now make a layer named back spirals and change the opacity to 20%. These are the large spirals in the background. I made them with the Large Spiral brush. Final shape layer is named back rings and is made with the rings brush set at a smaller diameter. This also has an opacity of 20%.

Add the Inner Shadow layer effect from your oval frame layer to the following layers, rings, tiny spirals and corner ring. If you don't know how see the thread Adding A Layer Effect To Other Layers. For the tiny spirals and corner ring you need to change the Distance and Size settings. To do that you would click on their layer and then click on the arrow on the layer. Now click where it says Inner Shadow. Change the settings in the Layer Style window.

Take your type tool and write your name. I used the font Univox. Make sure all your layers are locked in place, see green outline. And you're done. To save this signature use the save as a jpg in Photoshop setting found here, My settings.
