Click on the type layer with the name and apply this layer style to it...

Make a new layer and name it back shapes. Select the custom shape tool. Make sure you have Fill pixels selected in your options bar. See the thread Custom shape tool for more information. Choose a color and make the shapes in the background. When you're happy with your shapes change the opacity of that layer. I set mine to 50%. *Pay attention that your back shapes layer is in the same order in the layers palette as you see in picture below. Now click on your blue border layer and apply a Stroke effect to it using the settings in the picture below. And you're done. To save this signature use the save as a jpg in Photoshop setting found here, My settings.
