Make sure black is the foreground color in the toolbox. In the layers palette click on the background layer. Now add a Bevel and Emboss layer style to this layer. Use the settings below. When done, click on the name Gradient Overlay in the left hand column. The Gradient Overlay options window will now be showing. Use the settings below. To select the gradient, click on the arrow in the blue box and choose the Foreground to Transparent gradient.

When saving your graphic to use online you can use guides to crop it. Go to View and make sure both Rulers and Snap are checked. Then go to View>Snap To and make sure Layers is checked. Go to your layers palette and click on the background layer. Drag a guide to each side of your graphic. Using the crop tool, click down on the top left corner of the guides and then drag to the lower right corner of the guides. You can either double click within the bounding box around your graphic or click on the checkmark in the options bar to apply the crop. Save your graphic as a PNG and you're done.
