START With A New File Select your gradient tool and click on the grad in the options bar. In the Gradient Editor window choose the Black, White grad and click OK. While holding the shift key click at the bottom of your file and drag it to the top then release. Name this layer back grad.

Open your picture file. While holding the shift key click and drag the Background from the layers palette over your siggy and release. Reduce/Enlarge your picture then apply Filter>Sharpen>Sharpen to it. I named this layer back TED pic. Take your brush tool and set the Mode to Color in the options bar. With black as your foreground color paint over your picture. In the layers palette lower the opacity of this layer to 50%.

With white as your foreground color take your type tool and write your name. I used the font Academy Engraved. Apply this layer effect to it. I used black for the color.

Make a new layer and name it left design. With white as your foreground color take your custom shape tool using the settings you see and make your shape.

Make a copy of the left design layer and name this copy right design. With the move tool hold the shift key and drag the shape over. Apply the Stroke layer effect from the name/Teddy layer to the left and right design layers.

Make a new layer and name it frame. With the rectangular marquee tool make a selection.

Select the gradient tool and choose the Black, White grad. Click at the top of the selection hold the shift key and drag to the bottom of the selection and release. Deselect.

Apply these two layer effects to the frame layer. When looking at a Layer Style window if your layer has more than one effect applied to it you can click on the name of the other effect(s), blue arrow, and the window will change to the selected effect.
